Put the same penalties for fake tests as you would for a fake passport and that should curbs anyone's enthousiasm for this sort of things.

The actual airplane is not seen as a high-risk place for spread of COVID due to the air circulation. It is more likely for people to spread the disease while waiting in the queues in the airports on either side.
For many years, backpackers from Eastern Europe have made fake yellow-fever vaccination certificates so that they could cross African countries’ borders. It is not that they wanted to avoid the vaccine, it is just that this vaccine was very difficult to obtain in Eastern Europe, with only a handful of shots made available each year. Public health authorities around the world knew that some percentage of yellow-fever vaccine certificates would be fake, but this was not seen to threaten the overall effectiveness of vaccine requirements. I don’t see why the same can’t hold for COVID.
