I review all code it ever pushes, primarily because on occasion it nukes thousands of lines for no reason.

When I read the 'what actually helped' it's clearly decision fatigue they are managing.

Here's the real trick, you need to change how you're using AI. Have the AI do the decisions. Treat the AI like it's a new hire dev that doesnt get to push to nothing. That you then have AI review the changes, have AI build tests around, have AI do a security review.
